To qualify for your complimentary Visual Life Story you must agree to the following terms:
- You must complete the Visual Life Story within 14 days of starting it.
- You must leave 1feedback about your experience with Souls Remembered and the creation of the story.
- Only one story may be submitted for development by a company/individual at a time. If you are a company which has multiple end-of-life care workers who would like a story for a patient, please contact us using the information in the footer of our site.
1 You will be sent a link to a feedback form, to the same email that you submitted for the visual life story. If feedback is not received within 7 days of the completion of your visual life story, the story will be deactivated.
